This is the Bally Professional Arcade warranty registration card. It seems to have been used during a transitional period, as it is addressed in care of "Astro Vision." It is to be mailed to Bally Manufacturing Corporation, Consumer Products Division, Care of Astro Vision, P.O. Box 133, Garner, Iowa 50438.
  • Bally Limited Warranty Card - This B&W scan is only 1/4 of the card. It is the part that the owner fills out. This was the original version of the document that was archived. It is kept here for archival purposes.
